Job summary

ModivCare is seeking an RN Case Manager for pediatric home health in Lackawanna County. You will assess patients, coordinate care with families and physicians, and supervise LPNs and caregivers to ensure high-quality home care. A Pennsylvania RN license and CPR are required, with 1 year of nursing experience preferred.

The role offers a Monday–Friday schedule, 40-hour week and a company vehicle. Travel time is included in the workweek.

Qualifications

• Active Pennsylvania RN license (Required) – BSN preferred.
• Active CPR Certification.
• 1 year of professional nursing experience preferred.
• Valid driver’s license.
• Pediatric or home care experience preferred.
• Strong leadership and communication skills.
• Comfortable working independently in the field.

Responsibilities

• Complete patient assessments, including data collection.
• Develop and oversee pediatric care plans with physicians and families.
• Supervise and support LPNs and caregivers.
• Coordinate care with families and interdisciplinary teams.
• Ensure clinical documentation is accurate, timely, and compliant.

CareGivers America – NEPA is hiring a full-time RN Case Manager to support pediatric patients and families throughout Lackawanna County.You’ll serve as a trusted clinical partner to families, coordinate care, and help children receive high-quality support from the comfort of theirhome.

Your Role as aPediatric RN Case Manager:

• Develop and oversee pediatric care plans with physicians and families
