CareGivers America – NEPA is hiring a full-time RN Case Manager to support pediatric patients and families throughout Luzerne, Wyoming, and Sullivan County. You’ll coordinate care and help children receive high-quality home-based support.

This role requires an active PA RN license and CPR certification, with a preference for a BSN. You’ll work Monday–Friday, travel in the field, and receive a competitive salary with a company vehicle.

Formación

• Active Pennsylvania or Compact RN license (required) – BSN preferred.
• Active CPR Certification.
• 1 year of professional nursing experience preferred.
• Valid driver’s license.
• Pediatric or home care experience preferred.
• Strong leadership and communication skills.
• Comfortable working independently in the field.

Responsabilidades

• Complete patient assessments, including OASIS data collection.
• Develop and oversee pediatric care plans with physicians and families.
• Supervise and support LPNs and caregivers.
• Coordinate care with families and interdisciplinary teams.
• Ensure clinical documentation is accurate, timely, and compliant.
• Participate in quality improvement and staff education.
• Promote safe, high-quality care in the home.

CareGivers America – NEPA is hiring a full-time RN Case Manager to support pediatric patients and families throughout Luzerne, Wyoming, and Sullivan County. You’ll serve as a trusted clinical partner to families, coordinate care, and help children receive high-quality support from the comfort of their home.

Your Role as a Pediatric RN Case Manager:

• Complete patient assessments, including OASIS data collection
