ModivCare in Pennsylvania is seeking a dedicated RN Case Manager to support pediatric patients at home, coordinating with families and physicians to deliver high-quality, child-centered care. This full-time role covers Lackawanna County with a Monday–Friday schedule and a 40-hour workweek, travel time included, and a company vehicle provided.

You will assess patients, develop care plans, supervise LPNs and caregivers, and ensure accurate documentation while participating in quality improvement.

Qualifications

• Active Pennsylvania RN license (Required) – BSN preferred.
• CPR certification required.
• 1 year of professional nursing experience preferred.
• Valid driver’s license required.
• Pediatric or home care experience preferred.
• Strong leadership and communication skills.
• Comfortable working independently in the field.

Responsibilities

• Complete patient assessments, including OASIS data collection.
• Develop and oversee pediatric care plans with physicians and families.
• Supervise and support LPNs and caregivers.
• Coordinate care with families and interdisciplinary teams.
• Ensure clinical documentation is accurate, timely, and compliant.
• Participate in quality improvement and staff education.
• Promote safe, high-quality care in the home.

CareGivers America – NEPA is hiring a full-time RN Case Manager to support pediatric patients and families throughout Lackawanna County. You’ll serve as a trusted clinical partner to families, coordinate care, and help children receive high-quality support from the comfort of theirhome.

Your Role as aPediatric RN Case Manager:
