Traditional Home Health Care in Wilkes-Barre, PA is seeking an RN Case Manager to join our team. This full-time role involves coordinating patient care at home, assessing needs, and collaborating with physicians, nurses, therapists, and families to ensure high-quality outcomes.

Qualifications

• Active RN license in Pennsylvania.
• BLS certification.
• Valid driver’s license and reliable transportation.
• Strong assessment and documentation skills.
• Ability to work independently and manage time effectively.
• Excellent communication and interpersonal skills.
• Acute care and/or home health experience preferred.

Responsibilities

• Assess patients and develop personalized care plans with physicians, nursing supervisors, patients, and families.
• Provide hands-on care, administer medications, and evaluate patient progress.
• Maintain and submit timely documentation, including OASIS assessments.
• Collaborate with therapists, caregivers, and healthcare professionals to ensure comprehensive care.
• Educate patients and families on treatment plans and self-care techniques.
• Inform patients of additional services and resources as needed.
