Job summary

CenterCare Hospice in Colorado Springs is seeking a compassionate Registered Nurse to join our team. You will provide nursing care in the patient’s home in line with current standards to support comfort and quality of life for patients facing life-limiting illness.

The ideal candidate holds an active RN license, CPR/BLS, and Hospice/Home Health experience, with reliable transportation. You will collaborate with physicians and the care team to educate families, coordinate services, and deliver

Qualifications

• Must possess a Nursing Degree from an accredited college or university, or be a graduate of an approved RN program.
• Must possess a current, unencumbered, active RN license.
• Current CPR/BLS certification.
• Minimum 1 year of Hospice/Home Health experience.

Responsibilities

• Provide nursing care in the patient’s home in accordance with current standards and regulations.
• Assess patient and family needs to determine scope and frequency of services.
• Communicate effectively with physicians, care team, patients and families.
• Educate patient and family on disease, self-care, and end-of-life care.
• Coordinate diagnostic and therapeutic services as ordered by the physician.
• Document care plans and ensure compliance with policies and regulations.
• Review care plans to verify quality of care.
