Job summary

CenterCare Hospice is seeking a dedicated Registered Nurse to provide in-home nursing care for patients in Pueblo. The role emphasizes compassion, empathy, and high-quality end-of-life care in the patient’s residence.

The RN will coordinate with the care team, educate families, and ensure adherence to legal and clinical standards while supporting patients’ physical, emotional, and spiritual needs.

Qualifications

• Must possess a current RN license.
• Must have CPR/BLS certification.
• Minimum 1 year hospice experience.

Responsibilities

• Provide nursing care in the patient’s residence in accordance with applicable standards and regulations.
• Assess the caregiver’s ability to meet patient needs at admission and during care.
• Communicate with physicians, care team, patients and family members effectively.
• Educate patient and family about disease process, self-care techniques and end-of-life care.
• Requisition and arrange diagnostic and therapeutic services as ordered by physician.
• Follow drug administration protocol and document appropriately.
• Review care plans to verify appropriate care is rendered.

Communicate to the care teamEducationMust possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN programSpecific Requirements· Must possess a current, unencumbered, active RN license· Must have current CPR/BLS· Minimum 1 year of Hospice experience· Must have reliable transportation and proof of auto insurance coverage· Must pass a pre-employment background check and drug test
