Job summary

CURA-HPC EASTOK LLC is seeking a dedicated hospice nurse to evaluate and admit patients, administer medications, and implement care plans. You will communicate with patients, families, and physicians, document status, and participate in team meetings. A valid nursing license and driver’s license are required.

Qualifications

• Graduate of an accredited school of nursing (ADN/BSN).
• 1 year nursing experience preferred.
• Valid state driver’s license and auto liability insurance.

Responsibilities

• Evaluates and admits qualifying patients per Medicare eligibility.
• Administers medications and treatments per Hospice policy.
• Communicates effectively with patients, families and physicians.
• Develops and implements written Plan of Care with physician orders and Hospice guidelines.
• Documents patient status accurately and concisely as required by Hospice policy.
• Addresses patient needs and problems, nursing interventions and patient response, safety education.
• Promotes teamwork and participates in case conferences/team meetings.
• Makes visits as scheduled.

Addresses patient needs and problems, nursing intervention and patient response, safety interventions and education and teaching.Maintains a committed and cooperative attitude with staffing, promoting teamwork and harmony by readily assisting other staff and patients.
