Job summary

PASCO/SW Hospice in Colorado is seeking a compassionate Hospice RN to serve as a trusted Case Manager, delivering in-home care and guiding families through decisions with dignity.

You will perform comprehensive assessments, develop care plans, administer medications, document care, participate in team meetings, and provide emotional support. A valid CO RN license and 2+ years in hospice or home health are required; on-call rotation may be required.

Qualifications

• Colorado RN license is required.
• Minimum 2 years nursing experience; hospice/home health preferred.
• Valid driver's license and insured vehicle.
• Ability to work independently with patient-centered care.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans and revise as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
• Collaborate with an interdisciplinary team that shares commitment to excellence.
• Participate in on-call rotation as needed.

What You'll Do

What We're Looking For

• Current RN license in the State of Colorado
• Minimum of 2 years of nursing experience, hospice or home health experience preferred
• Homecare Homebase (HCHB) experience preferred, not required
• Valid driver’s license and reliable, insured vehicle
• Strong clinical skills and a passion for patient-centered care
• Ability to work independently and a passion for making a real difference

Salary: $57.00 - $59.00 per hour
