Homecare and Hospice of the Valley is seeking an RN Case Manager for our Glenwood Springs team. Schedule is Monday–Friday, 8:00 AM–5:00 PM with competitive hourly compensation and comprehensive benefits.

You will lead patient-centered care plans, coordinate services, and build strong relationships with patients and families, delivering compassionate, high-quality care in the field.

Formación

• 1 year of home health or hospice experience preferred.
• Strong organizational and communication skills.
• A heart for providing comfort, dignity, and exceptional care.

Compensation: $43.00-$49.00 per hour
