Job summary

Andwell Health Partners is seeking a Skilled Home Health Nurse (RN) to deliver high-quality nursing care in patients' homes across the Rumford to Buckfield area and surrounding communities. You will assess, plan, implement, and evaluate patient care with autonomy and teamwork, ensuring compassionate support for patients and families.

The role requires Maine RN licensure and at least 1 year of clinical experience (home health preferred).

Qualifications

• Active RN license in Maine.
• At least 1 year of clinical nursing experience (home health preferred).
• Ability to work independently with sound judgment and critical thinking.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide in-home skilled nursing care and visits.
• Perform holistic assessments beyond symptoms.
• Administer wound care, medications, and chronic disease support.
• Document care accurately to support better outcomes.
• Collaborate with interdisciplinary team to ensure quality care.
• Travel locally to patient homes within coverage area.
