Job summary

Moments Hospice in Minnesota is seeking an RN Case Manager to coordinate patient care across the care continuum and work with the clinical team to develop and refine care plans. You will ensure timely access to services and collaborate with physicians, social workers, and family members to support patients and their loved ones.

Qualifications

• Licensed Registered Nurse in the state you work in (MN).
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open communication with patients, families, and the healthcare team.
• Manage patient care coordination for caseload.
• Participate in interdisciplinary team meetings for care planning.
