Transitions At Home is hiring a Full-Time RN Case Manager in the Madison, Wisconsin area. You will coordinate and oversee patient care in home settings, ensuring personalized, high-quality outcomes for every client.

A compassionate and experienced RN is sought to join our close-knit team. The role offers a reasonable schedule, no weekend/on-call burden, and a suite of benefits including a company vehicle, health/dental/vision, 401k with match, and tuition reimbursement.

Qualifications

• Active Wisconsin Registered Nurse License.
• Two years' nursing experience, 1 year in Home Health preferred.
• CPR Certification.
• Valid Wisconsin Driver's License and Car Insurance.

Responsibilities

• Provide skilled nursing care including assessments, medications, treatments, and coordination of services in the patient’s home.
• Counsel patient and family/significant others in place of residence per plan of care.
• Under supervision of RN Clinical Manager and Administrator, ensure quality outcomes.

What does a Transitions at Home RN Case Manager do?

Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient's physician orders and plan of care under the direction and supervision of the RN Clinical Manager and Administrator.

Hours: Full-Time

Paid Drive Time - Company Vehicle

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• No Weekends or Holidays
• Limited on-call rotation
