Qualifications

• Active Wisconsin Registered Nurse License.
• Minimum of 2 years nursing experience; 1 year in Home Health preferred.
• CPR certification required.
• Valid Wisconsin driver’s license and car insurance.

Responsibilities

• Provide skilled nursing assessments, medications, and treatments in patients’ homes.
• Coordinate services and counsel patients and families in their residence.
• Collaborate with the RN Clinical Manager and Administrator to implement the care plan.

We're hiring! Transitions At Home, is currently seeking to hire a Full-Time RN Case Manager in the Madison area. Our growing home health agency is seeking a compassionate and experienced full-time RN Case Manager to join our team. In this role, you’ll coordinate and oversee patient care in the home setting, ensuring personalized, high-quality outcomes for every client.

What does a Transitions at Home RN Case Manager do?

Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient's physician orders and plan of care under the direction and supervision of the RN Clinical Manager and Administrator.

Hours: Full-Time

Paid Drive Time - Company Vehicle

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• Limited on-call rotation

Qualifications / Job Requirements
