Job summary

Transitions At Home for Southern Wisconsin seeks a compassionate CNA to provide in-home care in the Delavan area. Responsibilities include companionship, personal care, meal prep, light housekeeping, transportation, and safety monitoring to keep clients comfortable and safe at home.

The role supports the RN Case Manager under the RN Clinical Manager, with hours Monday–Friday 7:00 am–2:30 pm and competitive wage plus benefits, including 401k, FSAs, tuition reimbursement, PTO, and referral

Qualifications

• Must have Wisconsin CNA license.
• Must meet Medicare Conditions of Participation for Home Health Aide training.
• Compassionate and patient-focused care.
• Ability to read, follow directions, and communicate clearly.
• Mature and reliable with the demands of home health care.

Responsibilities

• Provide companionship and personal care to clients in their homes.
• Assist with meal preparation and light housekeeping.
• Assist with transportation and safety monitoring.
• Report changes in client condition to the RN Case Manager.
• Adhere to physician orders and plan of care under RN supervision.

Job description

Transitions At Home, is currently seeking to hire Compassionate CNA to care for a Client in the Delavan area.

This Caregiver will be responsible for providing companionship, personal care, meal preparation, light housekeeping, transportation, and safety monitoring to clients in their homes. The ideal candidate will be dependable, compassionate, and willing to assist with a variety of tasks to help clients remain comfortable and safe in their homes.

What does a Transitions at Home CNA do?

Transitions At Home CNA provides personal cares, bath and hands on care, household services. Reports change in condition and needs to the RN Case Manager in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Full-Time

• Monday through Friday 7am-230pm

Qualifications / Job Requirements:

• Must meet Medicare Conditions of Participation for Home Health Aide training program and competency.
• Must have a current Wisconsin CNA License.
• Have a sympathetic attitude toward the care of the sick and elderly.
• Ability to carry out directions, read and write.
• Maturity and ability to deal effectively with the demands of the job.
