Transitions At Home is a trusted provider of home health services, committed to delivering high-quality, compassionate care to patients in the comfort of their homes we are seeking an experienced RN Case Manager to support our care team and play a vital role in coordinating and delivering patient-centered care.

The RN Case Manager will oversee clinical and regulatory matters, provide guidance, coordinate services, and counsel patients and families in their place of residence under the direction

Qualifications

• Active Wisconsin Registered Nurse License.
• Two years’ nursing experience, 1 year in Home Health preferred.
• CPR Certification required.
• Wound Care experience beneficial.
• Valid Wisconsin Driver's License and car insurance.

Responsibilities

• Provide expert guidance and oversight in clinical and regulatory matters.
• Coordinate services and counsel patients and families in their home.
• Conduct staff training and care plan reviews.
• Investigate incidents under RN Clinical Manager supervision.

Transitions At Home is a trusted provider of home health services, committed to delivering high-quality, compassionate care to patients in the comfort of their homes we are seeking an experienced RN Case Manager to support our care team and play a vital role in coordinating and delivering patient-centered care.

What does a Transitions at Home RN Case Manager do?

Transition at Home RN will provide expert guidance and oversight in clinical and regulatory matters. This role includes staff training, critical thinking, care plan review, incident investigation, and who also coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Full - Time

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• Weekends or Holidays
• Limited on-call rotation
