Job summary

AMN Healthcare seeks an experienced RN Case Manager in Los Angeles, CA for a 13-week assignment. You will coordinate Care Continuum, discharge planning, and patient-focused care plans in collaboration with hospital teams.

The role requires CA RN license, BLS, LA City Fire & Safety, and 2 years of acute hospital care coordination. Cerner is used for documentation; shifts are 8 hours, days, with every-other-weekend coverage.

Qualifications

• Must hold CA RN license and BLS certification.
• Experience: 2 years recent acute hospital care coordination.
• Experience with CommonSpirit / Dignity Health is preferred.
• LA City Fire & Safety training is required.

LENGTH OF ASSIGNMENT– 13 weeks

SHIFT / HOURS PER WEEK– 8hr Days, EOW (40hrs/wk)
