Job summary

RN.com is seeking a dedicated RN Case Manager to deliver patient-centered, holistic care. You will assess patient and family needs, develop and implement individualized care plans, evaluate outcomes, and adjust interventions accordingly.

You will coordinate care across interdisciplinary teams, advocate for patients, and educate families throughout the continuum of care. This position requires a California RN license, CPR/BLS, and 3+ years of RN experience with 2+ years in case management.

Qualifications

• Active CA RN license required.
• CPR/BLS certification required.
• 3+ years RN experience, including case management.
• 2+ years in case management specifically.

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Provides patient-centered, holistic care by assessing patient and family needs, developing and implementing individualized care plans, evaluating outcomes, and adjusting interventions as needed. They serve as an advocate and educator for patients and families, coordinate care across interdisciplinary teams, facilitate communication and referrals, delegate appropriate tasks, and promote safe, efficient, and high-quality outcomes throughout the continuum of care.

MINIMUM REQUIRED QUALIFICATIONS –

• CA RN license
• CPR/BLS
• 3+ years recent RN experience working in RN role
• 2+ years recent RN experience working in case management

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8-430, every other weekend required
