Job summary

Bristol Hospice is seeking a dedicated RN Case Manager to join our team in Libertyville, IL. This full-time role offers a Monday–Friday, 8am–5pm schedule with salary range $95,000 to $100,000 and coverage across Lake and McHenry Counties.

Responsibilities include developing individualized care plans, providing one-on-one patient and family support, assessing needs, monitoring vitals, administering medications, educating families, documenting progress, and collaborating with interdisciplinary

Qualifications

• Active Registered Nurse license in good standing in Illinois.
• Excellent observational and communication skills, both written and verbal.
• CPR certification (current).

Responsibilities

• Develop and implement individualized care plans based on patient history.
• Deliver attentive, one-on-one support to patients and families.
• Conduct thorough assessments of physical, emotional, and spiritual needs.
• Monitor and document vital signs (temperature, pulse, respiration, blood pressure).
• Administer physician-ordered medications and treatments with empathy.
• Educate patients and families on care techniques and symptom management.
• Track changes in patient condition and respond promptly.
• Maintain up-to-date clinical notes reflecting patient progress.

Qualifications

On An Average Day You Will:(includes, but not limited to)

• Develop and implement individualized care plans based on current and prior medical history

• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned

What You’ll Bring to The Team

• A heart for hospice care and a commitment to exceptional service
• A valid Registered Nurse license in good standing in the state you will work in
• A valid driver’s license and reliable transportation
• CPR certification (current)
• Strong observational and communication skills—both written and verbal
