Job summary

Bristol Hospice in Bakersfield, CA is seeking a dedicated Registered Nurse for an hourly, full-time Case Manager role. You will advocate for patients and families, coordinate individualized care plans, and deliver compassionate, comprehensive end-of-life care.

Qualifications

• RN with valid license in California.
• Excellent written and verbal communication.
• Ability to assess physical, emotional and spiritual needs.
• Strong documentation and care coordination abilities.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver one-on-one support to patients and families, ensuring comfort, dignity, and understanding.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track changes in patient condition and respond accordingly.
• Maintain detailed clinical notes reflecting progress and quality of care.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Supportive duties as part of a compassionate, mission-driven team.
• Other duties as assigned.

QualificationsOn An Average Day You Will:

• Develop and implement individualized care plans based on current and prior medical history
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned

What You’ll Bring to The Team:

• A heart for hospice care and a commitment to exceptional service
• A valid Registered Nurse license in good standing in the state you will work in
• A valid driver’s license and reliable transportation
• CPR certification (current)
• Strong observational and communication skills—both written and verbal
• A flexible, team-oriented attitude and willingness to support additional duties as needed
