Job summary

Bristol Hospice Bakersfield, CA is seeking a Registered Nurse Case Manager to provide compassionate, whole-person hospice care. This full-time, hourly role focuses on advocacy, comfort, and support for patients and families, within a mission-driven team.

You will develop care plans, monitor vitals, administer treatments, and collaborate with interdisciplinary teams to ensure dignified end-of-life care. RN license in good standing and CPR are required; drivers license and reliable transportation

Qualifications

• Valid Registered Nurse license in the state you will work in.
• CPR certification (current)
• A valid driver’s license and reliable transportation
• Strong observational and communication skills—both written and verbal
• Willingness to support additional duties as needed

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• Contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned
