Job summary

Bristol Hospice in Cleveland (OH) is seeking a dedicated RN Case Manager to support patients and families on their hospice journey. You will develop individualized care plans, monitor comfort, coordinate with the interdisciplinary team, and provide compassionate in-home or facility care.

This full-time, day-shift role covers the West side counties and requires an active RN license, CPR certification, and reliable transportation.

Qualifications

• Valid RN license in good standing in Ohio.
• CPR certification (current) is required.
• Excellent communication and observational skills.
• Driver's license with reliable transportation.

Responsibilities

• Develop and implement individualized care plans based on patient history.
• Provide one-on-one care and support to patients and families.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor vital signs and document accurately.
• Administer physician-ordered medications and treatments with care.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Maintain up-to-date clinical notes reflecting patient progress.

• Develop and implement individualized care plans based on current and prior medical history
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned

What You'll Bring to The Team:

• A heart for hospice care and a commitment to exceptional service
• A valid Registered Nurse license in good standing in the state you will work in
• A valid driver’s license and reliable transportation
• CPR certification (current)
• Strong observational and communication skills—both written and verbal
