Job summary

Bristol Hospice is seeking a dedicated RN Case Manager in Sturgeon Bay, WI. You will develop personalized care plans, coordinate one-on-one support for patients and families, and monitor clinical progress to ensure comfort and dignity throughout the end-of-life journey.

The role requires a valid RN license, a compassionate, team-driven attitude, and strong communication skills. You will work Monday–Friday, 8am–5pm, in a supportive, mission-driven culture with comprehensive benefits.

Qualifications

• A valid Registered Nurse license in good standing in the state you will work in.
• Proven ability to develop and implement individualized care plans for patients.
• Strong observational and communication skills, both written and verbal.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver attentive, one-on-one support to patients and families.
• Conduct thorough assessments of physical, emotional, and spiritual needs.
• Monitor and document vital signs accurately.
• Administer physician-ordered medications and treatments with empathy.
• Educate patients and families on care techniques and symptom management.
• Track and communicate changes in patient condition for timely responses.
• Maintain up-to-date clinical notes and collaborate with interdisciplinary teams.

• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned

What You'll Bring to The Team:

• A heart for hospice care and a commitment to exceptional service
• A valid Registered Nurse license in good standing in the state you will work in
• A valid driver's license and reliable transportation
• CPR certification (current)
• Strong observational and communication skills-both written and verbal
