Job summary

Bristol Hospice in Joliet, IL is seeking an RN Case Manager to provide compassionate, patient- and family-centered hospice care. You will advocate for patients, develop individualized care plans, and coordinate with interdisciplinary teams to ensure comfort and dignity throughout the care journey.

The role emphasizes advocacy, education, and timely communication, with opportunities for professional growth within a mission-driven, family-centered hospice environment.

Qualifications

• A valid Registered Nurse license in good standing in the state you will work in.
• A valid driver’s license and reliable transportation.
• CPR certification (current).
• Strong observational and communication skills—both written and verbal.
• A flexible, team-oriented attitude and willingness to support additional duties as needed.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Provide one-on-one support to patients and families, ensuring comfort and dignity.
• Conduct assessments of physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs accurately.
• Administer physician-ordered medications and treatments with empathy.
• Educate patients and families on care techniques and symptom management.
• Track changes in patient condition and respond promptly.
• Maintain up-to-date clinical notes reflecting patient progress.

• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding

• Administer physician-ordered medications and treatments with skill and empathy

• Keep detailed and up-to‑date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned
