Job summary

Bristol Hospice in Green Bay, WI is seeking an experienced RN Case Manager to advocate for patients and families on the journey of hospice care. This full-time daytime role emphasizes compassionate, whole-person support and collaboration with a mission-driven team.

You will develop personalized care plans, monitor patients’ conditions, and work with interdisciplinary colleagues to deliver comprehensive end-of-life care in a supportive environment.

Qualifications

• Valid RN license in good standing in the state you will work in.
• Current CPR certification.
• Strong communication and compassionate patient care.
• Ability to develop and implement individualized care plans.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver one-on-one support to patients and families, ensuring comfort and dignity.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs; administer physician-ordered meds with empathy.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.

QualificationsOn An Average Day You Will:

• Develop and implement individualized care plans based on current and prior medical history
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned

What You’ll Bring to The Team:

• A heart for hospice care and a commitment to exceptional service
• A valid Registered Nurse license in good standing in the state you will work in
• A valid driver’s license and reliable transportation
• Cpr certification (current)
• Strong observational and communication skills—both written and verbal
• A flexible, team-oriented attitude and willingness to support additional duties as needed
