Qualifications

• Licensed RN in Michigan is required.
• BSN preferred.
• Valid Michigan driver’s license with proof of auto insurance and reliable transportation.
• 1–2 years of acute care or skilled nursing experience; certified home health experience preferred.
• Passion for delivering high quality care to older adults.

Join Our Team as an RN Case Manager for home visits in beautiful downtown Holland! Atrio Home Care, the partnership of Holland Home and Resthaven, is seeking a compassionate RN Case Manager to join our Grand Rapids Holland/Lakeshore team. This position will involve providing direct care to clients in Ottawa and Allegan Counties.

Responsibilities

• Perform initial and comprehensive assessments and deliver compassionate care to clients

FTE: Full or part-time opportunities available

Schedule/Shift

• Monday - Friday
• Day shift 8-5 pm, with flexibility in scheduling to help you maintain a healthy work/life balance
