Job summary

Atrio Home Care, a partnership involving Holland Home, Brio and Rest Haven, seeks a compassionate LPN to provide in-home care across the Grand Rapids area. You will deliver skilled nursing services, follow Plans of Care, and educate clients and caregivers to promote independence.

This full-time or part-time role offers day shifts with a focus on high-quality one-on-one care and thorough documentation using Matrix EMR on iPads.

Qualifications

• Current Michigan LPN license.
• Valid driver’s license, reliable transportation, and proof of auto insurance.
• 1–2 years of acute care or skilled nursing experience (home health preferred).
• Passion for delivering exceptional care to older adults.

Responsibilities

• Deliver skilled nursing care under RN supervision, including wound care, dressing changes, catheter care, and medication/injection administration.
• Follow and support individualized Plans of Care in collaboration with physicians and the care team.
• Provide specialized nursing services such as preventive care (skin care, pressure relief) and chronic disease management.
• Educate clients and caregivers on disease processes, self‑care, and health promotion to encourage independence.
• Maintain accurate, timely documentation using the Matrix EMR system (iPads provided).

Atrio Home Care— a partnership between Holland Home, Brio and Rest Haven— is seeking a compassionate Licensed Practical Nurse (LPN) to provide in-home care throughout the Grand Rapids territory. This role focuses on delivering high-quality, one‑on‑one care to clients in their homes within approximately a 40-mile radius of Grand Rapids.

Key Responsibilities

• Deliver skilled nursing care under RN supervision, including wound care, dressing changes, catheter care, and medication/injection administration
• Follow and support individualized Plans of Care in collaboration with physicians and the care team
• Provide specialized nursing services such as preventive care (skin care, pressure relief) and chronic disease management
• Educate clients and caregivers on disease processes, self‑care, and health promotion to encourage independence
• Maintain accurate, timely documentation using the Matrix EMR system (iPads provided)

Schedule & Location

Territory: Grand Rapids team

Schedule: Full-time or part‑time opportunities available

Hours: Day shift (8:00 AM – 5:00 PM)

Weekend/On-Call/Holidays: None
