Job summary

Atrio Home Care in Grand Rapids is seeking an RN Case Manager to perform home visits within a 40-mile radius. Monday–Friday, day shift 8–5 with schedule flexibility for work-life balance. Compensation includes bonuses for quality outcomes and premium pay for weekend/holiday visits.

Qualified candidates are licensed RNs in Michigan, with BSN preferred, and a valid MI driver’s license. 1–2 years in acute care or skilled nursing, plus home health experience, is ideal.

Qualifications

• Licensed RN in Michigan; BSN preferred.
• Valid Michigan driver's license and proof of auto insurance.
• 1–2 years of acute care or skilled nursing experience; prior home health preferred.
• Passion for delivering high quality care to older adults.
• Paid Time Off (PTO) and benefits are offered.

Responsibilities

• Perform initial and comprehensive assessments and deliver compassionate care to clients.
• Collaborate with physicians and care team to develop a Plan of Care.
• Promotes client and family/caregiver independence through counseling and education.
• Identify barriers to desired outcomes and evaluates outcomes of care.
• Prepare clinical documentation and progress notes using iPads for the EMR system.
• Participates in on-call rotation.
