Job summary

Atrio Home Care in Grand Rapids is seeking a compassionate Licensed Practical Nurse (LPN) for in-home care across the Grand Rapids territory. You will provide one-on-one nursing services in clients’ homes within roughly a 40-mile radius, under RN supervision.

The role offers day shifts (8:00 AM – 5:00 PM) with options for full-time or part-time schedules and a supportive care team dedicated to high-quality, autonomous patient care.

Qualifications

• Current Michigan LPN license required.
• 1-2 years acute care or skilled nursing experience (home health preferred).
• Valid driver's license and reliable transportation.
• PTO provided and 403(b) with access to a professional financial planner.
• Provide meaningful, one-on-one care and make a real impact.

Responsibilities

• Deliver skilled nursing care under RN supervision, including wound care, dressing changes, catheter care, and medication/injection administration.
• Follow and support individualized Plans of Care in collaboration with physicians and the care team.
• Provide specialized nursing services such as preventive care (skin care, pressure relief) and chronic disease management.
• Educate clients and caregivers on disease processes, self-care, and health promotion to encourage independence.
• Maintain accurate, timely documentation using the Matrix EMR system (iPads provided).
