Job summary

Atrio Home Care in Holland, MI is seeking an RN Case Manager for home visits across Ottawa and Allegan Counties. You will assess clients, develop Plans of Care with physicians, and educate families to foster independence. The role includes on-call rotation, and full- or part-time options with weekday schedules and premium pay for weekend visits.

Licensed RN in Michigan required; BSN preferred. 1–2 years acute/Skilled Nursing experience; valid driver’s license and reliable transportation a must.

Qualifications

• 1–2 years of acute care or skilled nursing experience.
• Certified home health experience preferred.
• Licensed RN in Michigan; BSN preferred.

Responsibilities

• Perform initial and comprehensive assessments and deliver compassionate care to clients
• Collaborate with physicians and other care team members to develop a Plan of Care
• Promotes client and family/caregiver independence through counseling and education
• Coordinate client discharge planning
• Identify barriers to desired outcomes and evaluates outcomes of care
• Prepare clinical documentation, progress notes in a timely manner using iPads for the EMR system
• Participates in on-call rotation
