Qualifications

• Licensed RN in Michigan is required.
• BSN preferred.
• Valid Michigan driver’s license with proof of auto insurance and reliable transportation.
• 1–2 years of acute care or skilled nursing experience; certified home health experience preferred.
• Passion for delivering high quality care to older adults.

Responsibilities

• Perform initial and comprehensive assessments and deliver compassionate care to clients.
• Collaborate with physicians and other care team members to develop a Plan of Care.
• Promotes client and family/caregiver independence through counseling and education.
• Identify barriers to desired outcomes and evaluate outcomes of care.
• Prepare clinical documentation and progress notes in a timely manner using iPads for the EMR system.
• Participates in on-call rotation.
