Job summary

Moments Hospice is seeking an RN Case Manager to provide compassionate, patient-centered care on an on-the-road schedule across Minnesota. You will assess patient needs, plan and deliver nursing care, educate patients and families, and coordinate with a multidisciplinary team to ensure high-quality end-of-life care.

Responsibilities:

• Conduct initial client assessments and continuously refine care plans.

Qualifications:

• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance
