Job summary

Moments Hospice is seeking an RN Case Manager to deliver compassionate, patient-centered care on the road. You will assess patients, develop and refine care plans, and coordinate with families and the care team to ensure comfort and quality in final days and weeks.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.

Responsibilities:
