Job summary

Moments Hospice is seeking an experienced RN Case Manager to provide compassionate, patient-centered care during patients' final days and beyond. You will conduct assessments, develop and adjust care plans, and coordinate with families and the care team to ensure seamless service.

The role requires an RN license, proficient EMR use, strong communication, and the ability to travel as needed. A company car with fuel and insurance coverage is provided as part of a competitive benefits package.

Qualifications

• Licensed RN in the applicable state.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to travel and drive as required.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.

Responsibilities:
