Brookestone Home Health & Hospice, part of the Vetter Senior Living family, is seeking an RN Case Manager to provide in-home nursing care, coordinate services, and advocate for patients and families. You will work with an interdisciplinary team to deliver high-quality care across our service areas.

Qualifications

• Current Registered Nurse (RN) license.
• Current CPR certification.
• Strong assessment, critical thinking, and clinical decision-making skills.
• Excellent communication and care coordination abilities.

What You'll Do
