Job summary

Homecare and Hospice of the Valley in Glenwood Springs seeks a Bilingual Spanish RN Case Manager to provide patient-centered care and coordinate services.

Schedule: Monday–Friday, 8:00 AM–5:00 PM. Compensation: $43.00–$49.00 per hour. Utilize English and Spanish to support patients and families.

You will lead care planning, collaborate with a multidisciplinary team, and deliver compassionate care in home/hospice settings.

Qualifications

• Active Colorado Nursing License.
• Bilingual in English and Spanish.
• 1 year of home health or hospice experience preferred.
• Strong organizational and communication skills.

Responsibilities

• Lead the coordination of patient-centered care plans.
• Build meaningful connections with patients and families.
• Provide autonomous field nursing with team support.
