Job summary

Hospice-of-the-Valley,-Inc in Glenwood Springs, CO is seeking a caring bilingual Registered Nurse Case Manager (English/Spanish) to join our team. Schedule: Monday–Friday, 8:00 AM–5:00 PM; compensation: $43–$49 per hour.

You will lead coordination of compassionate, patient-centered care plans in the field with autonomy and support from an experienced team. Requirements include Active Colorado Nursing License, bilingual in English and Spanish, and 1 year of home health or hospice experience

• 1 year of home health or hospice experience preferred.
• Strong organizational and communication skills.

Responsibilities
