Job summary

Hinds LifeCare in California’s Central Valley seeks a Full Time RN Case Manager to deliver skilled palliative and end-of-life care in the community. As part of the interdisciplinary team, you will help develop an integrated care plan and coordinate hospice services for patients and their families.

We value a nursing background with NLN-accredited program, RN licensure, CPR, and at least one year in acute care or community health; hospice experience is a plus.

Qualifications

• Completed a professional nursing program accredited by the National League for Nursing (prefer completion of a baccalaureate program in nursing).
• One year experience in acute care facility or community health agency, hospice experience a plus.
• Current RN Licensure and CPR Certification.
• Valid driver’s license, auto insurance, reliable transportation and a willingness to use personal vehicle in the course of employment.
• Ability to meet the physical demands of the job (Push, Pull, Lift/Carry 35 lbs.)
• Available to work 8:00am-4:30pm M-F with on call rotation.

• Provide skilled palliative and end-of-life care in the community setting.
• Facilitate development of an interdisciplinary care plan and ensure coordination and medical management of hospice services for patients and families.
