Job summary

Hinds LifeCare seeks a Full Time RN Case Manager to deliver skilled palliative and end-of-life care in patients' homes within California’s Central Valley. The role coordinates an interdisciplinary plan and ensures medical management of hospice services for patients and families.

The ideal candidate will have an NLN-accredited nursing background, RN licensure with CPR certification, and the ability to travel locally.

Qualifications

• Completed a professional nursing program accredited by the National League for Nursing (prefer completion of a baccalaureate program in nursing).
• One year experience in acute care facility or community health agency, hospice experience a plus.
• Current RN Licensure and CPR Certification.
• Valid driver’s license, auto insurance, reliable transportation and a willingness to use personal vehicle in the course of employment.
• Ability to meet the physical demands of the job (Push, Pull, Lift/Carry 35 lbs).
• Availability to rotate weekends.

• Promote the philosophy of hospice and support for patients and their loved ones.
