The RN Case Manager is an experienced Registered Nurse who provides skilled palliative and end-of-life carein the community setting. As a member of the Interdisciplinary Team, the RN Case Manager facilitates thedevelopmentof a collaborative interdisciplinary plan and ensures the overall coordination, collaboration andmedical management ofhospice services for patients and their significant others.
The RN Case Manager is also responsible for promoting the philosophy of hospice, which redefines hope byfocusing on quality of life and connection with loved ones during a patient’s final days, rather than on curing a disease.
