Job summary

Moments Hospice is seeking an RN Case Manager to support patients at the end of life, working across multiple settings as needed. You will assess needs, develop care plans, and coordinate services with an interdisciplinary team to ensure compassionate, patient-centered care.

The role requires an active RN license, EMR proficiency, strong communication skills, and reliable transportation with insurance. A meaningful, autonomous position offering growth opportunities awaits you on the road.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
