Job summary

Moments Hospice is seeking an experienced RN Case Manager to coordinate comprehensive, client-focused care in a mobile, on-the-road role. You will assess patients, create and adjust individualized care plans, and work closely with families and the clinical team to ensure quality outcomes.

Responsibilities:

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.

Qualifications:

• Licensed as a Registered Nurse / RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance
