Job summary

Moments Hospice in Minnesota is seeking an experienced RN Case Manager to lead patient care in the field. You will assess needs, coordinate services, manage medications, and support families through end-of-life care.

You will work Monday–Friday with a fleet car program and comprehensive benefits. This role offers leadership access and opportunities to shape your career while maintaining reasonable caseloads.

Qualifications

• Active RN license in the state you work.
• Proficiency with EMR systems.
• Strong organizational and communication skills.
• Ability to work independently with high standards of care.
• Compassionate, patient- and family-centered approach.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:0O PM
