Job summary

Moments Hospice is seeking an experienced RN Case Manager to coordinate patient care, assess needs, and develop care plans within a compassionate, interdisciplinary team.

This on-the-road role includes a company car with fuel and insurance, comprehensive health, dental, and vision insurance, generous PTO, and 401(k) with company matching.

Requirements: active RN license, EMR proficiency, strong planning and communication, and a valid driver's license with a reliable vehicle.

Qualifications

• Active RN license in the state of practice.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to work autonomously and manage caseloads.
• Valid driver''s license and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.

Responsibilities:
