Job summary

Moments Hospice in Wisconsin is seeking an experienced RN Case Manager to lead clinical care for a caseload of patients and families. You will assess needs, coordinate services, manage symptoms, and support families through end-of-life decisions.

This full-time, field-based role offers a salary of $75k–$90k, a fleet car program, comprehensive benefits, PTO, and 401(k). You’ll work with leadership in a supportive team, with reasonable caseloads and predictable scheduling.

Qualifications

• Active RN license in the state you work.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• A compassionate, patient- and family-centered approach.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
