Job summary

Moments Hospice is seeking an experienced RN Case Manager to deliver compassionate, patient-centered care and coordinate individualized care plans in collaboration with patients and families.

You will work closely with the owners to promote entrepreneurial growth, with on-call support and opportunities for clinical advancement. We offer a fleet car for business and personal use, generous PTO, and solid health benefits.

Qualifications

• Licensed as an RN in the state you work in.
• Proficiency with EMR software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.

Responsibilities:
