Job summary

Moments Hospice is seeking an RN Case Manager to deliver compassionate, patient-centered care across the care continuum. You will conduct assessments, develop and refine care plans, educate patients and families, and collaborate with the interdisciplinary team while driving coordinated, high-quality end-of-life care.

• Licensed as a Registered Nurse in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
