Job summary

Moments Hospice in New Jersey is seeking an experienced RN Case Manager to deliver compassionate, patient‑centered care in patients’ final days, weeks, and months. You will work closely with the company’s owners to foster internal growth and clinical advancement while maintaining high standards.

You will conduct assessments, refine care plans, and coordinate care across a broad healthcare team. The role includes a company car, competitive benefits, and a supportive work environment.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver’s License and reliable vehicle with current insurance
