Moments Hospice is seeking an RN Case Manager to lead clinical care for patients and families in the field. You will manage symptoms, coordinate care, and provide guidance during end-of-life care, with direct access to leadership and opportunities to shape your career.

The role emphasizes teamwork, manageable caseloads, and a fleet car program with covered gas and insurance for business and personal use.

Qualifications

• Active RN license in the state you work in.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• A compassionate, patient- and family-centered approach.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update an individualized plan of care with patients and families.
• Provide skilled nursing care, including pain and symptom management, medications, and comfort.
• Educate patients and caregivers on disease progression and end-of-life care.
• Coordinate care across your caseload and act as liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure holistic care.

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:0O PM

• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care

Qualifications
