Job summary

Baptist Health in Montgomery, AL seeks an RN - Case Management Team Lead to coordinate case management across all ages. You will supervise staff, monitor performance, and promote interdepartmental education to improve patient outcomes.

Collaborate with the Case Management Manager and Director, Medical Staff, and Administration to meet KPI targets and optimize resource use within Baptist Health's mission and values. This role may include orientation and staff development duties.

Qualifications

• Degree in Nursing; Bachelor’s degree preferred.
• Two to three years hospital Case Management experience required; care coordination and transitions preferred.
• Knowledge of community health care and socioeconomic resources.
• Joint Commission and third party payor requirements preferred.

Responsibilities

• Coordinate and supervise case management for all age groups.
• Monitor and manage employee performance; foster cross-department communication.
• Collaborate with Manager, Director of Case Management, Medical Staff and Administration to meet KPI targets and optimize resources.
• Provide orientation, education, and allocate human resources in the area.
• Additional duties within scope may be assigned.
