Job summary

Qualifications

• Active RN license.
• Two to three years of hospital Case Management experience.
• Experience in care coordination and transitions of care preferred.
• Knowledge of community health care resources.

Responsibilities

• Coordinate and supervise case management for all age groups.
• Monitor employee performance and provide education on case management activities.
• Collaborate with Manager, Director, Medical Staff and Administration to meet KPI targets and optimize resource utilization.
• Orient and allocate human resources in the assigned area.
